How it Works
Who can participate in the Onkwehón:we Health Check-In?
To be eligible, you must meet the following criteria:
Be at least 16 years old
Live on Six Nations
OR
Belong to Six Nations AND live in any of the following locations:
Brant
Hamilton
Niagara
Haldimand-Norfolk
(Old version)
(New version)
Both versions of the coupon are accepted for referrals!
Geographic Range of Survey
Six Nations of the Grand River, Ohsweken ON
Referral program
If you complete a survey, you’re eligible to refer up to 3 family members or friends through our coupons. Receive a $25 gift card for each referral that successfully completes the survey. The coupon is NOT required to complete the survey.
LHIN (Local Health Integrated Network)
For the purpose of the survey, we are excluding Halton.
Scheduling & Drop-ins
If you are an eligible participant and would like to participate, feel free to drop-in (first come, first serve basis) or schedule a survey!
Location
Iroquois Office Suites, located in the Ohsweken Plaza - 1721 Chiefswood Rd.
Hours
Drop-in: Monday - Friday 9:00 AM - 2:15 PM
Referral gift card pickup: Monday - Friday 9:00 AM - 4:00 PM
What are the possible benefits for me and/or the Six Nations community?
The survey is estimated to take between 1.5 and 2 hours to complete. As a token of appreciation for your valuable time, you will receive a $50 gift card upon completion of the survey and $25 for each referral (Maximum of 3 referrals x $25 = $75). If you agree to participate, your information and experiences will contribute to our understanding of community health and wellness and help Six Nations Health Services and SNGR to provide better services in the future.
